AUTHORIZATION TO DATE AND COMPLETE INSTRUMENT
Payee is authorized by each of the makers and by all the indorsers, sureties, and guarantors, without notice to any of them, to date this instrument as of the day when the first disbursement of the loan evidenced by this instrument is made and to fill in any blank spaces in this instrument to conform to the terms on which the loan evidenced by this instrument is made.
This is a UCC form. It is intended to be an addendum to other forms.
